Care Management Program

A Home Care Nurse will assess, monitor, and help facilitate your ability to maintain living at home.  Care Management services may include the following; however, the program will be tailored to address individual needs:

An initial comprehensive nursing assessment of your physical and psychosocial status, which will include an evaluation of the skin, cardiovascular, respiratory, neurological, musculoskeletal, gastrointestinal and renal system.

  1. Special attention will be given to activity tolerance and functional status as they relate to level of self care.

  2. Current nutritional status will be evaluated.

  3. Current health conditions will be reviewed.  You will provide a medical history.

  4. Current medications will be reviewed.

  5. A safety evaluation of your home will be conducted.

  6. Emotional and cognitive status will be evaluated.

  7. The assessment will include physical evaluation, observations and interview.

  8. Adaptive equipment needs can be assessed.

  • Create a plan to include areas of concern, plans of action, goals and assessment of results.

  • A weekly home visit for assessment of medications compliance, general health status and associated needs.

  • A weekly weight and vital sign measurement.

  • Coordination of additional home care assistance as identified and requested.

  • Coordination of treatment with current physicians.

  • Telephone reports or emails to appropriate family designee as needed.

  • A monthly written report to a designated family member which outlines status, verification of hours/time spent with care management and suggestions where appropriate.

These services would be scheduled and provided during the normal hours of business operation Monday through Friday, 8:30 AM to 4:30 PM.  In event of an emergency, the on-call Surrey staff member can be reached by cell phone,  however, be aware that Care Management services are not intended to be an alternative for emergency medical care and treatment. The Care Management Program is under the supervision of a staff nurse.  The charges for the Care Management Program are as follows:
